Position Summary: As a Social Services Assistant, you will assist in planning, developing, organizing, implementing, evaluating, and directing social service programs in accordance with current existing federal, state, and local standards, as well as established policies and procedures, to assure that medically related emotional and social needs of the resident are met/maintained on an individual basis.
Responsibilities
•Assist in planning, organizing, implementing, evaluating and directing the social service programs of this facility
•Assist in developing assessments and written plans of care to identify the needs of each resident
•Ensure that all components of the care plan are being followed by reviewing MDS’s and nurses’ notes and attending meetings
•Effectively communicate information concerning a patient’s condition with other departments and government agencies
•Effectively schedule and run care conference meetings
•Participate in the discharge planning, providing information to residents and families of programs available
•Seek out new methods and make recommendations on changes in policies and procedures to the Social Services Director or Administrator based on current federal and state regulations
•Create and uphold an atmosphere of warmth, patience, enthusiasm, and a calm and cheerful environment
•Under direction of Social Services Director, our Social Services Assistant Social Services Coordinator will assist residents and families in adjusting to placement, illness, and plan of care to attain the highest practicable level of functioning in accordance with current federal and state regulations, and Company policies and procedures
•You will assist residents and families wishes regarding treatment and assist with resident and family transition and adjustment to the facility; participation in resident and family council meetings, assist with coordination of appointments, transportation and other resources as requested
•You will also support Admissions and assist with discharge planning services
Qualifications
•High school diploma or equivalent
•Ability to work with physically and mentally ill, disabled, and emotionally upset elderly residents and families
•Ability to organize, prioritize and meet deadlines
•Ability to maintain confidentiality and be discreet
•Must be knowledgeable of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care
•Experience with geriatric and mental health population
•Associate or bachelor's degree preferred
•Creative problem-solving skills and strong communication skills
•Experience with PointClickCare (PCC) - Electronic Medical Record preferred
