Role Overview: As a Dementia Care Specialist , you will provide personalized, evidence-based interventions and support strategies for seniors with dementia. Working closely with families and caregivers, you’ll implement activities, communication techniques, and environmental modifications that promote well-being and independence. Key Responsibilities: Develop and implement individualized care plans addressing cognitive and behavioral needs. Train caregivers and family members in effective communication and therapeutic activities. Monitor patient progress and adjust interventions to maintain engagement and reduce anxiety. Collaborate with social workers, nurses, and therapists to coordinate comprehensive care. Maintain accurate documentation and ensure compliance with best practices. Qualifications: Background in gerontology, social work, nursing, or occupational therapy. Experience working with patients who have dementia or Alzheimer’s disease.
